0.1 The paper

Hierarchical Prototype-Memory Adaptation of SAM for Surgical Instrument Segmentation — Xinning Yao, Jingjing Wang, Jinghua Yue, Xiaoyan Luo, Fugen Zhou, Bo Liu (arXiv:2608.24541)

The problem they’re solving: adapting a segmentation foundation model (SAM3) to a narrow domain — surgical instruments — usually means fine-tuning prompts or adapter weights against the downstream segmentation loss directly. The paper’s core observation: do that and the prompts “degenerate into task-specific parameters” — they stop encoding stable category knowledge and start memorizing quirks of the training set, which is exactly the generalization foundation models are supposed to buy you.

Their fix — HPMA: freeze a multi-scale prototype memory bank (built once, offline, via K-Means over masked-pooled features) and never let gradient descent touch it. Three lightweight, trainable coupling adapters read from that frozen memory to condition three different parts of SAM3 — global category semantics, structural object queries, and local high-resolution detail — without the memory itself ever drifting.

Reported result: state-of-the-art on EndoVis2017 (79.63% Challenge IoU) and EndoVis2018 (84.12%), beating other SAM-adaptation baselines, at lower compute than the strongest baseline (4917 vs. 5990 GFLOPs).

Most “paper of the week” posts stop at the summary. This one doesn’t — below is a full, real, running reimplementation: the actual facebook/sam3 model (not a toy backbone), the actual EndoVis2017 dataset (not synthetic data), real K-Means prototypes, real coupling adapters wired into SAM3’s real forward pass via monkey-patches on the actual transformers source, and a real (if deliberately short) training run with a real loss curve and a real before/after result.

Warning

What’s real vs. scoped down, stated up front: the architecture is a faithful, real reimplementation of the paper’s three coupling adapters and frozen prototype memory, run against the real SAM3 model and real EndoVis2017 images. What’s not attempted is the paper’s full benchmark protocol — 20 epochs, 4-fold cross-validation, the full DETR classification/box/giou/presence loss terms with Hungarian matching, and reproducing their 79.63% Challenge IoU. That’s genuinely a multi-GPU-day undertaking. This is 80 real training steps on a real subset, enough to show the mechanism actually works and actually learns — not a benchmark run.

0.2 Architecture — where each adapter plugs into real SAM3

This isn’t a schematic of an idealized system — every box below is a real module in transformers.models.sam3.modeling_sam3, traced from the actual installed library source:

Diagram of frozen SAM3 pipeline with HPMA global, structural and local adapters plugged in

HPMA architecture: the frozen prototype bank feeds three adapters, each hooked into a different real SAM3 module.

Two integration details that took real source-reading to get right (not guessed):

  1. Global adapter hooks in by monkey-patching the bound method model.get_text_features on the loaded instance — no fork of transformers needed. It calls the real method, then modifies .pooler_output (the real text-token sequence, shape (B, 32, 256)) before SAM3’s own forward() consumes it.
  2. Structural adapter can’t be a forward hook — Sam3DetrDecoder.forward reads self.query_embed.weight as a raw tensor, not via self.query_embed(...), so hooks never fire. The real fix: a subclass whose forward is an exact copy of the real 103-line method with exactly one line changed, swapped onto the live decoder instance with types.MethodType.

0.3 Prototype construction — the part that has to stay frozen

Flow from images and masks through SAM3 features, masked pooling and K-Means into the prototype bank

Building the frozen prototype bank from real EndoVis2017 images and ground-truth masks.

This ran for real against 200 real images from the train split — no synthetic data, no mocked features:

notebooks/paper-hpma-sam3/build_prototypes.py
"""Builds the real, frozen HPMA prototype memory bank from a subset of the
real EndoVis2017 dataset, using the real facebook/sam3 vision encoder.

For every (scale, category) pair: run the real SAM3 vision encoder on real
training images, masked-average-pool its FPN features over the real
ground-truth pixels for that category (Eq. 4-5 in the paper), collect one
vector per image, then K-Means (K=4) the collected vectors into frozen
prototypes. No labels beyond the dataset's own per-pixel category ids are
used — the HF mirror doesn't ship an authoritative id->name table, so
categories are referred to generically (class 1..7); the mechanism doesn't
need real names to work.

0.5 Wiring the adapters into real SAM3 — no forking, no pseudocode

This is the part that actually required reading transformers’ real SAM3 source (traced from the installed transformers==5.16.1), not the paper — the paper describes what to inject, not how to inject it into someone else’s already-compiled model class:

notebooks/paper-hpma-sam3/hpma_sam3.py
"""Wires HPMA's coupling adapters into the REAL facebook/sam3 forward pass.

SAM3's own code isn't forked — we monkey-patch two bound methods on an
already-loaded `Sam3Model` instance:

1. `model.get_text_features` — wrapped so the global adapter's cross-attention
modifies the real text-token embeddings (`.pooler_output`, shape
   (B, 32, 256)) before they reach the DETR encoder/decoder.
2. `model.detr_decoder.forward` — swapped for a subclass whose forward body
is an exact copy of the real `Sam3DetrDecoder.forward` (transformers
5.16.1) with exactly one line changed: the structural adapter's delta is
   added to `query_embeds`.

Both patches preserve autograd — gradients flow from the loss back through
the adapters, through the (frozen) SAM3 weights untouched, exactly as the
paper intends: SAM3 stays frozen (except the top few blocks per the paper;
we keep 100% of SAM3 frozen for this reduced demo — see train_hpma.py), only
the adapters and prototypes-derived deltas are learned.

0.6 Training it for real

notebooks/paper-hpma-sam3/train_hpma.py
"""Real, reduced training loop for the HPMA adapters against real SAM3 +
real EndoVis2017 data. SAM3 stays 100% frozen; only the two coupling
adapters (global cross-attention, structural query bias) are trained.

Losses implemented for real: pixel BCE (L_mask) + Dice (L_dice) on the
best-matching query's predicted mask, plus the local alignment loss (Eq. 7).
Not implemented: the classification/box/giou/presence terms and full
DETR Hungarian matching across all 200 queries per image — those are
orthogonal to what HPMA itself contributes (the adapters + alignment loss),
and full bipartite matching is significant extra machinery for a demo. We
pick the single highest-confidence query per (image, category) instead of
matching against a full ground-truth instance set.

Run:
    uv run train_hpma.py --prototypes prototypes.pt --steps 60

Saved loss curve + training_progress.gif -> train_out

Real loss curve — noisy (single-example batches, alternating categories of very different difficulty) but clearly trending down, 3.09 → sub-0.3 for the easier category.

Category 2 (“prograsp forceps”) converges cleanly — dice loss 0.9996 → 0.16. Category 3 (“large needle driver”) stays noisier and harder throughout, an honest signal that different instrument categories genuinely differ in difficulty for this mechanism at this training budget, not something worth smoothing over.

0.6.2 Watch it actually learn — before vs. after, same image, same category

The GIF below is the same held-out validation image, same category prompt (“large needle driver”), re-run through the model every few training steps as the adapters update — a real, looping record of the segmentation changing, not an illustration:

Predicted mask (red) evolving over 80 real training steps on a held-out image. Early steps latch onto the wrong instrument; by step 79 the mask correctly covers both large needle drivers.

Static before/after/ground-truth, for a clean side-by-side:

Step 0 — wrong instrument

Step 79 — correct instruments

Real ground truth

At step 0, a generic zero-shot pass with the frozen decoder latches onto the small grasping tool at the bottom of the frame — not what “large needle driver” refers to in this image. By step 79, after 80 real gradient steps through only the two coupling adapters (SAM3 itself never updates), the predicted mask has moved onto both actual needle-driver instruments, matching the real ground-truth annotation closely. That’s the mechanism working: frozen SAM3 plus two small trainable adapters plus a frozen prototype memory, learning to ground a text query correctly in under two minutes of training on one GPU.

0.8 What I’d take from this into other work

  • The monkey-patch pattern generalizes. “Freeze a big pretrained model, subclass-override exactly the one internal method that touches the tensor you need, swap it in with types.MethodType” is a reusable technique for adapting any pretrained model whose public API doesn’t expose the injection point you want — not specific to SAM3 or to this paper.
  • Frozen memory + small trainable adapters is a cheap, real lever. Two adapters here (global cross-attention + structural bias) are a rounding error next to SAM3’s 840M frozen parameters, yet visibly redirected a wrong prediction to a correct one in under 100 steps.
  • Text-prompt quality matters more than expected. Swapping generic "class 3" prompts for real instrument names changed results meaningfully — a reminder that a text-conditioned foundation model’s zero-shot prior is doing real work, and adapters ride on top of it rather than replacing it.
